- Microsoft Excel format order form - Right click

and choose "Save Target As" to download
(preferred form)
- Microsoft Word format order form - Right click
here
and choose "Save Target As" to download
Once you have filled out the form, please print. If you do
not have a printer, please save the Word or Excel file and email it to
admin@converttapes.com.
Then, mail your videos and order form to: (Email
admin@converttapes.com for
address). The current turnaround time is less than 5 days. If
you need to make a payment, go to the Pricing page and use the "Buy Now"
buttons to assist in making payments. You can also include your
payment with your tapes.
